Health Forms medical bag

A health form is the most recent report of your child's last physical exam and contains his or her record of immunizations. It is usually required by camps or schools before your child attends. Most camps and schools require that your child have had a physical examination within the 12 months preceding attendance. If you want your child's entire medical record (different from a health form) because you are relocating or for any other reason, you may click here to go to that part of our web page.

Please use this page to request a health form for your child. We will try to complete your child's health form as soon as possible but please understand that during the busy seasons before camps and schools begin, it will take at least 15 business days (3 weeks) to process your request. In an emergency, we will process your request in 3 business days. We do charge a fee for health forms, as follows:

If you want to send a check, click here for a printer safe copy of the request form that you can fill out and mail to us with your check. Please note on the check the first and last name of the child for whom you are requesting a health form.
